Hmmm, this is an interesting idea. In my previous district, we had a checklist of interventions that the teacher or support staff had previously tried. The possible interventions covered the front and back of the page. The teachers were against this, as they felt it could somehow be used against them. Then again, the climate of that district was problematic.
We never had an outcome form. We assigned specific tasks to members of the IST at the meeting, and these tasks were included in the minutes (which were distributed via e-mail). In addition, we delineated a time period during which these tasks would get accomplished, as well as a date for review.

I can see how teacher's can get defensive about what interventions they have tried. This year we are encouraging teachers to come to IST earlier before they try a million things to get specific strategies.
We do assign different tasks to team members now as needed (e.g. observations, consultation, etc) and we have a form for that. But I thought it would be nice to give the teacher's some outcome form they can take take with them- like to check off strategies like "graphic organizer" or write in the specific techniques we suggested.
